Can a bio-impedance probe measure back pain objectively? A 2024 clinical study says yes.

Jang S, Ho JG, Jo A-R, et al. Assessing Pain Levels Using Bioelectrical Impedance in Low Back Pain Patients: Clinical Performance Evaluation. Diagnostics 2024;14:2447 (MDPI, open access).

Researchers at Soonchunhyang University and Yesan Myongji Hospital in Korea tested the 182 Hz bio-impedance probe that 3i Bot™ deploys, on 105 adults — 45 healthy volunteers and 40 patients with diagnosed low back pain, plus a separate age check. With the probe on the lumbar muscles for 15 seconds at a time, three electrical readings (resistance, impedance and phase angle) were clearly higher in painful backs, and told painful from healthy backs with 96–98% accuracy (sensitivity 0.93–0.95, specificity 0.93–0.96).

  • What was measured. Resistance (R), impedance (Z) and phase angle (PA) of the lumbar paraspinal muscle between L1 and L5, with the probe set low (3.5 V, continuous) so it measured rather than treated. Pain was scored on the VAS, Oswestry and Roland–Morris scales.
  • Results. Low back pain vs healthy: R 11.55 vs 8.97 Ω, Z 33.97 vs 29.92 Ω, PA 0.15° vs 0.09° — all highly significant. Cut-offs of R ≥ 10.09 Ω, Z ≥ 31.89 Ω or PA ≥ 0.12° flag a painful back. Readings tracked how much pain patients reported (correlation with VAS ≈ 0.75; with Oswestry ≈ 0.68). Age did not change the readings.
  • Why 182 Hz. Earlier impedance studies at 50–100 kHz reached only 55–93% accuracy. At the low 182 Hz frequency the current is shaped mainly by the extracellular matrix, so the fibrosis, fat infiltration and inflammation of painful muscle stand out.
  • What it does not yet do. The probe separated painful from healthy backs cleanly but did not grade mild from severe pain within the pain group, cannot identify the cause of the pain from a single frequency, and the study pooled acute and chronic pain. The authors call for multi-frequency and longer-term studies.
  • What it means for 3i Bot™.
